DATA ^PLUS  Service 

The Center for Clinical Informatics can help you with all of your data capture and reporting needs. 

Organizations may contract with the Center for Clinical Informatics for the DATA^PLUS service.  The service includes the use of any of the free questionnaires available through ACORN, or organizations may utilize any other questionnaire of their choosing (copy right permitting). The DATA^PLUS service can  be customized to meet the unique needs of client organizations, including private labeling through use of organization logos on questionnaires, reports, and web sites.
